Big News for Primary Care Access in the Great River Region

The Great River Ontario Health Team is pleased to share a $5.1M+ investment through Ontario’s Primary Care Action Plan, helping to connect up to 13,687 residents to a primary care provider across our region.

This investment means more individuals and families will gain access to ongoing, coordinated care closer to home—supporting Ontario’s goal of connecting everyone to primary care by 2029.

This work is being delivered through a collaborative effort between Seaway Valley Community Health Centre, the Centre de santé communautaire de l’Estrie, the Glengarry Nurse Practitioner Led Clinic, and the Rideau St. Lawrence Family Health Team.

Together, these partners are already working as one team—and will continue to do so—to expand access to interprofessional primary care across the region, bringing together family physicians, nurse practitioners, nurses, social workers, dietitians, pharmacists, and physiotherapists to support comprehensive, connected care.

As intake processes roll out, priority will be given to residents without a primary care provider, including those registered through Health Care Connect in eligible communities in the Great River region.

A strong example of what collaboration can achieve—better access, stronger coordination, and more connected care for thousands of residents in our communities.
