While our OHT will address diseases/health conditions of various population groups in our attributed population in future years, our initial work will focus on the following three Project Team areas:

Building Capacity with Primary Care Providers

This Project Team will identify ways to increase preventative care cancer screening opportunities and support primary care providers to identify and implement innovative opportunities.

Mental Health, Addictions and Substance Use Health

This Project Team will work together to identify gaps in services and strive to ensure streamlined and timely access to mental health, substance use health and addiction services in the most appropriate setting.

Frail Older Adults and Complex Care Needs

Together, this Project Team will work collaboratively to find ways to improve coordination and supportive care in the community so that people can access care in a timely manner in the most appropriate setting.
