  • Successful hiring of the Executive Transformation Lead.
  • Launch of Physician and Primary Care Association.

Summer 2022

  • Permanent name selected – Great River Ontario Health Team.
  • Several support documents submitted to MOH/OH.

Spring 2022

  • Launch of Lived Experience Partners Table, French Language Services Table, the Primary Health Care Clinicians Table and a Website Development Work Group.

April 2022

  • Collaborative Quality Improvement plan submitted to MOH.

March 2022

  • Transfer Payment Agreement allowing for hiring of OHT staff.

Winter 2021

  • Establishment System Transformation Tables (Digital Health, Quality, Equity, Evaluation and Education and HR, Finance and Legal).

September 2021

  • Official announcement approval of UCCA OHT as a Cohort 3 OHT.

Summer 2021

  • Collaborative Decision-Making Agreement (CDMA) developed with 30 partners as signatories.
  • Governance structure established – Collaboration Council and Steering Committee.
  • Cornwall Community Hospital selected as the Fundholder for the OHT Implementation Funds.
  • Consensus Decision-Making Framework developed.

January to April 2021

  • Development and submission of combined application under the name “Upper Canada, Cornwall and Area OHT”.

November 2020

  • Both partner groups are deemed as “in development” OHTs and asked to merge.

May 2019

  • Readiness self-assessment submitted by both Cornwall and Area partners and Upper Canada partners.
