Vision

The overarching intent for our Ontario Health Team is to bring together sector partners, providers and those with first-hand experience with services and care, with aims to plan, coordinate and improve service provision for the future.

As a collective, we will become accountable for the care experiences, population-health outcomes and costs of care for the people and communities we serve.

Through our work together, we value collaboration, innovation, person-centredness and equity and remain mindful of our vision:

Guiding Principles

The guiding principles to shape the design of the Great River OHT are aligned with the Quintuple Aim:

1.

Better patient and population health outcomes

  • We commit to continuous quality improvement by measuring and monitoring our patient and population health status to drive evidence-based change and optimize performance.
  • We will deploy efficient and innovative approaches and models of care that produce jointly desired outcomes.
  • We recognize that wholistic health and well-being is influenced by more than simply health status and biology; we will adopt a broad-based approach factoring the social determinants of health and culturally informed processes to help drive better patient and population health outcomes.
  • We will improve population health education by partnering with organizations whose educational expertise can support advancement of health literacy, which is key to improving disease prevention and overall wellness.

Better patient, family and caregiver experience

  • We will meaningfully engage, partner and build shared system transformation with patients/clients/residents, care partners, families and community members.
  • We value person-centred care that responds to each individual’s needs across their lifespan and within a context of empathy, compassion and action.

    • We embrace shared leadership and teamwork by strengthening collaborations and integrating opportunities for collective system/services planning, decision-making and implementation of programs.
    • We believe that our providers are our assets and their experience matters. We commit to system reform to ensure our providers are supported to thrive and find joy in their personal and professional lives.

    Better value

    • We commit to deliver efficient and sustainable services by learning from others and by testing innovative models of care designed to achieve this goal. New models that meet efficiency and sustainability benchmarks will be extended and incorporated into ongoing programs across the OHT.

      Advancing Health Equity

      • We recognize the need to address inequities caused by unjust and unfair factors within our health care system.
      • We believe that focusing on individuals and communities who need improvement the most will lead to a reduction of disparity and correction of systemic inequities to improve the conditions of daily life for everyone.

        Our OHT is committed to establishing standards of care and services that recognize and respond to our communities’ diverse range of values and beliefs to ensure the equitable allocation of health care services.
