Health Care Connect: The First Step to Getting a Family Doctor or Nurse Practitioner in Your Community

The Great River Ontario Health Team is encouraging residents across the region to take the first step toward accessing a family doctor or nurse practitioner by registering with Health Care Connect, Ontario’s provincial program that helps people find a primary care provider in their community.

Being connected to a primary care provider ensures:

  • Comprehensive, long-term, high-quality care
  • Health promotion and disease prevention
  • Access to disease screening and early detection programs
  • Ongoing management of chronic conditions to improve quality of life

Registering is easy and won’t affect your existing care. Residents no longer need to unregister from their current provider. By registering, Health Care Connect can identify those seeking care closer to home and help plan for local community needs.

HOW TO REGISTER:

Ontario is taking steps to ensure people and their families have access to high-quality care closer to home for generations to come. Ontario’s Primary Care Action Plan will connect two million more people to a publicly funded family physician, nurse practitioner or primary care team by 2029, achieving the government’s goal of connecting everyone in the province to primary care.
