Ontario Health Teams Helping More People Access Primary Care
Ontario Health Teams (OHTs) are working across the province to connect more people with a family doctor, a primary care team, and the essential care they need. As part of the Ontario government’s Primary Care Action Plan, these teams are helping to connect 300,000 more people to primary care this year.
To make this happen, Ontario is funding 78 new and expanded interprofessional primary care teams. These teams include family doctors, nurse practitioners, social workers, and other professionals who work together to provide better, faster care—close to home.
OHTs are leading this work in local communities. Their main focus this year is to:
- Strengthen local networks of care providers,
- Help bring new primary care teams to life, and
- Match people without a doctor—especially those on the Health Care Connect waitlist—with available care in their area.
OHTs are also helping improve services for people with chronic conditions and those who need home care, so that care is more connected and easier to navigate.
Thanks to the leadership of Ontario Health Teams and their local health partners, more Ontarians will have access to the care they need—when and where they need it most.
