SUMMARY:Community Voices Collective: Shaping Better Health Care Together
DESCRIPTION:The Great River Ontario Health Team (GR OHT) is inviting community members from across Akwesasne\, Cornwall\, Stormont\, Dundas\, Glengarry\, Russell Township\, and parts of rural Southeast Ottawa to help shape the future of health services through the launch of the Community Voices Collective. \nGuided by the tagline “Shaping better health care together\,” the initiative will create new opportunities for residents to share their experiences\, perspectives\, and ideas to help inform how services are planned and improved across the region. \nThe Community Voices Collective is designed to be flexible and accessible. Community members can choose to participate in ways that work for them\, including: \n\nShort surveys\nOccasional focus groups or conversations\nCommunity engagement opportunities and pop-up events\n\nParticipation is entirely voluntary\, with no long-term commitment required. \n“Everyone has experiences with the health and social service system\, and those experiences matter\,” said Carilyne Hébert\, Engagement and Navigation Lead with the GR OHT. “We want to ensure that the people who use these services have opportunities to help shape how they evolve and improve in the future.” \nThe initiative builds on the work of the GR OHT’s Lived Experience Partners Table (LEPT)\, a group of community members who help guide and inform health system priorities based on their lived experiences. \nThe GR OHT is especially hoping to hear from individuals with diverse backgrounds and experiences\, including rural residents\, newcomers\, youth\, seniors\, caregivers\, and members of equity-deserving communities. \nCommunity members who join the Community Voices Collective may occasionally be invited to participate in surveys or focus groups related to health and social services in the region. Some engagement opportunities may include small tokens of appreciation\, such as gift cards or entry into prize draws. \nResidents interested in joining the Community Voices Collective can register online here:\nforms.office.com/r/ghgTL4780d

SUMMARY:Big News for Primary Care Access in the Great River Region
DESCRIPTION:The Great River Ontario Health Team is pleased to share a $5.1M+ investment through Ontario’s Primary Care Action Plan\, helping to connect up to 13\,687 residents to a primary care provider across our region. \nThis investment means more individuals and families will gain access to ongoing\, coordinated care closer to home—supporting Ontario’s goal of connecting everyone to primary care by 2029. \nThis work is being delivered through a collaborative effort between Seaway Valley Community Health Centre\, the Centre de santé communautaire de l’Estrie\, the Glengarry Nurse Practitioner Led Clinic\, and the Rideau St. Lawrence Family Health Team. \nTogether\, these partners are already working as one team—and will continue to do so—to expand access to interprofessional primary care across the region\, bringing together family physicians\, nurse practitioners\, nurses\, social workers\, dietitians\, pharmacists\, and physiotherapists to support comprehensive\, connected care. \nAs intake processes roll out\, priority will be given to residents without a primary care provider\, including those registered through Health Care Connect in eligible communities in the Great River region. \nA strong example of what collaboration can achieve—better access\, stronger coordination\, and more connected care for thousands of residents in our communities. \nRead MPP Nolan Quinn’s announcement:\nhttps://nolanquinnmpp.ca/ontario-connecting-13687-more-people-to-primary-care-in-sdg/ \n📸 From left to right:\nAmanda Nixon\, Co-Interim Executive Director and Clinical Lead Nurse Practitioner\, Glengarry Nurse Practitioner-Led Clinic\nMelissa Brown\, Co-Interim Executive Director and Administrative Assistant\, Glengarry Nurse Practitioner-Led Clinic\nJustin Towndale\, Mayor\, City of Cornwall\nDiane Plourde\, Executive Transformation Lead\, Great River Ontario Health Team\nErin Killoran\, Executive Director\, Seaway Valley Community Health Centre\nLyne Martineau\, Executive Director\, Centre de santé communautaire de l’Estrie\nNolan Quinn\, MPP\, Stormont-Dundas-South Glengarry\nDr. Marilyn Crabtree\, Clinical Lead\, Great River Ontario Health Team\nElizabeth Whitten\, Executive Director\, Rideau St. Lawrence Family Health Team\nTracy Crowder\, Project Manager\, Great River Ontario Health Team
